The Regulations of the New Brunswick College of Pharmacists requires your official name to be recorded on the register of the College. Pharmacy professional’s Certificates of Registration should be in their official name.
To submit a name change request, please have electronic versions of the following documents ready to upload:
- Canadian government-issued photo ID (ex: Driver’s license or passport)
- Copy of marriage certificate (if this is the reason for change)
